I have 15 years experience as a GP in South Shields. I am able to offer appointments at short notice and usually at a price that is significantly cheaper than your own GP can offer for HGV and Taxi medicals. If you require medicals for other types of industry then please ask to see if I can meet your requirements?

The examination
The examination
The examination will last approximately 20 minutes. Please remember the following?
- Summary Printout (brief or intermediate) of your medical record from your GP surgery and any relevant hospital letters (if you are currently under a specialist) so I can establish details of any treatment you have had and medication you are taking.
- The summary should include all current and past medical conditions and the last Three year's consultations (ie: starting from when you were last seen). If you take blood pressure medication, the summary should include your last 3 readings. If you are diabetic then you should include your most recent bloods, foot and eye screening and verification of no recent hypoglycaemic or hyperglycaemic episodes. Please bring reports of any heart investigations you had had? It is best to submit the request in writing to your GP practice reception. Please ensure that the date of issue is no less than 2 weeks old?
- Photocard Drivers Licence for I.D purposes
- Forms for taxi drivers should be obtained from the local authority and checked to make sure that any registered doctor can perform the medical and not just your GP? D4 Forms for HGV and PCV drivers (D4) can be downloaded from the DVLA website.
